Maine Maritime Academy Swim Trip

WHEN: Saturday, December 6, 2003

WHO: Students in grades 3 through 8

COST: $10.00 per person

BUS SCHEDULE:
Bus leaves DIS Elementary School at 9:15am and returns at 2:30pm
This is a school bus trip and subject to the same school behavior code.

WHAT TO BRING TO THE BUS:

• Exact cost of trip ($10.00)
• Permission slip/release form - YOU MUST BRING YOUR RELEASE FORM WITH YOU TO THE BUS. Do NOT give your release form to the teacher.

An "All You Can Eat Buffet Lunch" will be provided in the Academy Cafeteria.

Space is limited, so sign up early. Reservations must be made no later than Wednesday, December 3rd. Call Dick McKenney at 348-9949, or fill out the signup form below.

CHAPERONES ARE NEEDED!
