Great Skates Trip

!! IMPORTANT !! - Parents MUST send children to school with a note for school and a release form for Rec Board. Your child will not be able to go on this trip without BOTH documents! Send child to school with note for the school - authorization for child to stay after school for the Great Skates trip AND send child to school with a signed release form for the Rec Board in order to board the bus.

WHEN: Friday November 17, 2006

WHO: Students in grades 3 - 8

COST: $8.00 per person

BUS SCHEDULE:
Bus leaves DIS Elementary School at 12:45pm and returns at approximately 6:00pm

WHAT TO BRING:
A note from parent for child to stay after school for trip (give this note to teacher) You will not be allowed to go on trip without this note.
Exact cost of trip ($8.00)
Permission slip/release form due upon bus entry. You will not be allowed to go on trip without this signed release form.
Skates or money for skate rental (skates are $1.50, rollerblades are $3.00)
Laser tag fee ($3.00)
Money for food or a bag lunch
Money for concessions and games
